An epoxy floor coating comes in two parts, a resin and a hardener, mixed just before it goes down. Once it cures it forms a hard, glossy layer bonded to the concrete underneath, which is why people choose it for garage floors, basements and workshops.

Mostly the concrete underneath. Epoxy holds onto the floor only as well as the surface lets it, so oil and grease have to come off, a smooth or sealed slab has to be roughened, and cracks have to be filled first. Moisture rising through the slab is the other common cause of failure, which is why the floor gets checked before any coating goes on.
The concrete gets checked for cracks, oil stains, old sealer or paint, and signs of damp.
A square of plastic sheet taped to the slab for at least 16 hours shows whether moisture is coming up through the concrete.
Oil and grease come off, and the surface is ground or otherwise roughened so the coating has something to grip.
Cracks, chips and holes are filled and levelled before any coating goes on.
The two parts are mixed and rolled out in the coats the product calls for. Flakes or an anti-slip additive can go into the coating for texture and grip.

No. A lot of what's sold as epoxy floor paint is a one-part acrylic paint with a little epoxy added. A two-part epoxy coating is mixed on site and cures into a harder, thicker film. Thin or poorly bonded coatings are the ones that lift under hot tires when a car parks after a drive.
Concrete poured on the ground can pass moisture up from below. If too much comes through, it pushes the coating off the floor as bubbles or peeling patches. That's why the slab gets checked before anything is applied.

Most epoxies yellow over time in direct sunlight. Where the sun reaches the floor, such as a garage that sits with the door open, a UV-stable topcoat like polyaspartic or polyurethane is normally used over the epoxy.
A smooth epoxy floor can be slippery when it's wet. Flakes scattered into the coating, or an anti-slip additive in the topcoat, add grip.
